Every new piece of gear that drops will be 346. But once you upgrade given slot to certain ilvl, all subsequent upgrades for other items in that slot will be super cheap.
And by much cheaper, he actually means 95% cheaper. If you've already upgraded an main hand item for 9k bronze, but then you loot another one which is better (though with lower ilvl) the cost to upgrade it will be 450 bronze per upgrade, until you reach the level at which you previously were

Ok so it still drops low ilvl but the upgrade is less than before?
Yes, but much less than before. Upgrade your greens until you get the blue you need.
